Data Transfer Form
The American Association of Zoo Keepers, Inc. encourages all zoos and other animal care facilities to utilize the following data transfer form whenever they are shipping an animal between facilities. Providing this information to the receiving facility will ease the transition of a new animal into the collection and provide vital information to receiving staff. Previously there were three different forms to fill out. These have been combined into one form which encompasses information from the three previous forms. This form is four pages long.
All collection managers are asked to include the form where
applicable whenever an animal is shipped. THREE copies of
the form should be sent with the animal being shipped. At the receiving institution
these copies should be distributed to the following staff:
a) Curator
b) Keeper who will be caring for the animal
c) Zoo files and/or veterinarian
We all seek to provide the best and most professional care possible for the
animals in our collections. Using the AAZK data transfer form will
help provide receiving institution staff with the tools to make this possible.
